The folate most people are taking

You've taken it every day for months. You still feel like this.

Bone-deep tired by mid-afternoon. A head like cotton wool. Flat, for no reason you can name. If you are already taking a folate and nothing has changed, it may not be the dose that is wrong. It may be the form.

A woman sitting at a kitchen table in late afternoon light, resting her jaw on her hand, looking past the camera.

First, the ten-second version

Tick the ones that sound like you.

  1. You wake up tired after seven or eight hours.
  2. Your head feels thick by mid-afternoon.
  3. You lose words in the middle of a sentence.
  4. You are flat, and there is no reason for it.
  5. You have been told your bloodwork is normal.
  6. You take a multivitamin, a B-complex or a prenatal.
  7. You have taken folate for months and felt nothing change.
  8. Or you felt something, and it was worse rather than better.
  9. Someone has mentioned MTHFR to you.
  10. You have stopped expecting a supplement to do anything.

Three or more? Then the next four minutes are worth your time. There are seven reasons a folate supplement does nothing, and the first one is on the back of the bottle you already own.

01 · The form

You are not taking folate. You are taking a starting material.

Turn your bottle over. If the panel says folic acid, you are not taking folate. You are taking a synthetic compound your body has to convert — through four enzyme steps — before a single cell can use any of it.

Folic acid was synthesised in 1943 and it is in your multivitamin because it survives a warehouse. That is what it was chosen for. It is not the molecule your body runs on.

The molecule your body runs on is 5-MTHF. It is the end of the relay. L-Methylfolate is that finished molecule, already made, in a capsule.

Figure 1 Two routes to the same molecule

  5. MTHFR5-MTHFthe folate your cells use

Route B · what L-Methylfolate does

  1. L-Methylfolatewhat is in this capsule
  2. 5-MTHFthe folate your cells use

The gate. MTHFR is the enzyme at that last step — the one the gene is named after. Route B lands on the same molecule without ever reaching it.

Simplified — folate metabolism has more branches than this. These are the steps that decide the form. [1]

02 · The gene

Roughly one in two people cannot run that conversion at full speed.

The last step of the relay is run by an enzyme called MTHFR. The gene that builds it comes in more than one version, and the common variant slows the enzyme down.

One copy and the enzyme runs at roughly two thirds speed. Two copies and it is closer to a third. Across North America, Europe and Australia, somewhere between 40 and 60 per cent of people carry at least one copy. [2] [3]

Not a rare disease. Not something anyone would think to test you for. Just a bottleneck at the exact step that turns the folic acid in your multivitamin into something usable — sitting in about half the population, unnoticed.

  • ~30 one copy
  • ~58 neither

The enzyme still works in every row. What changes is how quickly it finishes the last step.

Left: midpoints of published ranges, not a measurement — around 20–40% of white and Hispanic Americans carry one copy, and roughly 8–20% of people across North America, Europe and Australia carry two. It is far less common in people of African descent. Right: enzyme activity measured in laboratory conditions — a measure of an enzyme, not of how anyone feels, and not a prediction about an individual. [2] [3]

03 · The dose

400 micrograms was never going to move anything.

Look at what your bottle actually contains. Most folate on the shelf is 400 mcg — a figure set to prevent a deficiency across a whole population, not to change anything in one person.

The published clinical research on L-methylfolate did not use 400 mcg. It used 15 milligrams. When the same research tested half that dose, it did not separate from placebo. [9]

15 mg is 37½ times a 400 mcg dose. That is not a bigger version of the same thing. It is a different product.

15 mg is 37½ times a 400 mcg dose. On the label that reads 25,000 mcg DFE, 6,250% Daily Value. This is a deliberate high-potency dose, printed plainly on the front rather than buried in a blend. [9]

04 · The stack

You are getting folic acid all day, and you did not choose it.

Since 1998 every enriched grain product in America has been fortified with synthetic folic acid. Bread. Pasta. Cereal. Crackers. Tortillas.

The first enzyme in the relay, DHFR, is slow in humans and saturates easily. [6] Past a single dose of roughly 300 to 400 mcg, folic acid starts appearing in the blood unconverted — researchers call it UMFA. [7] [8]

It does not wait politely for its turn. Unmetabolised folic acid circulates and competes for the same transport your body uses to move the active form to where it is needed.

So: a fortified breakfast, a fortified sandwich, and a multivitamin on top of both. All of it queuing at the same bottleneck.

Above ~0.8 mgper dayBlood levels climb faster than the dose does

Bar length is illustrative, not a measured curve. What unmetabolized folic acid means for long-term health is still debated; we are not going to tell you it is settled. [6] [7] [8]

Every step in a pathway is a chance to lose something. The shortest route through has the fewest.

05 · The folate trap

And if you tried a methylfolate and felt worse — that is a real thing.

Some people take a methylfolate for the first time, feel wired or headachy or irritable within a few days, decide it is making them worse and stop. That reaction has a name and it is not in your head.

5-MTHF hands its methyl group to homocysteine through an enzyme that needs vitamin B12 to work. If your B12 is low, that handoff stalls and folate backs up behind it. That is the folate trap.

The answer is not a "complex" with a token amount of B12 buried in it at a dose too small to matter, sold to you as a second active ingredient. The answer is to know your B12 status and to know exactly what is in your capsule. Ours has two ingredients and both are printed on the front of the bottle. If you need B12, take B12 — from something that tells you how much.

  1. 1 · The handoff5-MTHF gives up a methyl group. That is the whole job of the molecule.
  2. 2 · The currencyMethionine becomes SAMe — the methyl donor almost every other methylation reaction draws on.
  3. 3 · What it pays forMethyl groups go to DNA, to myelin, and to the pathways that build neurotransmitters.

Simplified. 5-MTHF is the main folate circulating in blood and the folate form that moves into the brain. [4] [5]

06 · The timeline

You gave it two weeks.

Folate works through methylation, and methylation is not a stimulant. Nothing arrives in forty minutes and nothing is supposed to.

Most people who notice anything describe it over three to six weeks, and what they describe is unglamorous. The afternoon does not collapse. The word arrives. It is Wednesday and you are still functioning.

That is why the guarantee here runs 60 days rather than 30. One bottle is 60 capsules and 60 days, and you can send it back on day 59.

A man standing at a kitchen counter in late afternoon light, both hands on the stone, head lowered, taking a moment.

Nothing about this is exotic. It is the ordinary machinery that decides whether a Tuesday afternoon is something you get through or something you endure.

Why the form is worth four minutes

07 · The label

And everything else in the capsule was never for you.

Pick up any bottle and read past the active ingredient. Magnesium stearate. Silicon dioxide. Microcrystalline cellulose. Titanium dioxide. Those are flow agents and fillers. They exist so a manufacturing line runs faster.

Ours has one other named ingredient. Glycine, 220 mg, doing the job of a carrier. That is the whole formula.

Two hands holding a plain unlabelled supplement bottle up to a kitchen window to read the panel on the back, with the Clean Health Lab bottle on the counter.
Figure 7What the words on your own label mean
  1. 1Turn the bottle over to the Supplement Facts panel.
  2. 2Find the line that says Folate.
  3. 3Read the words in brackets after it. That is the form.

  • (6S)-5-methyltetrahydrofolatethe finished form, written outReady to use
  • Quatrefolic · Metafolinbrand names for the finished formReady to use

Folinic acid is a real folate and a legitimate ingredient; it simply still has a step to go. None of these are dangerous, and none of them are disqualifying. The only question this table answers is which of them arrives finished. [1]

You do not have to finish the bottle to qualify and you do not have to explain yourself. Three to six weeks is when most people notice anything, so the guarantee is built to outlast that.

Questions

The six people actually ask.

Do I need a gene test first?

No. A test tells you your genotype; it does not change what the finished form is. Plenty of people test out of curiosity and plenty never do. It is not a prerequisite for choosing a folate.

Do I need to take B12 with this?

Possibly, and it is worth knowing rather than guessing. The enzyme that moves the methyl group off 5-MTHF is B12-dependent, so if your B12 is low the handoff stalls — that is the folate trap in reason 05. We do not put a token amount of B12 in the capsule and call it a complex; we publish two ingredients so you can see exactly what you are taking and add what you actually need. Ask your provider to check B12 and folate together.

Is this the same as the folate in my multivitamin?

Almost certainly not. Most multivitamins use folic acid. Turn the bottle over and read the panel — if it says folic acid, that is the starting material, not the finished form.

How long before I notice anything?

Most people who notice something describe it over three to six weeks. Some notice nothing. Individual responses vary and we are not going to pretend to know yours in advance — which is why the guarantee runs 60 days.

Is 15 mg too much?

It is a deliberate high-potency dose and it is printed on the front of the label rather than buried in a blend. It is the strength used in the published clinical research. Whether it is right for you is a conversation with your provider.

What if it does nothing for me?

Send it back. The 60-day money-back guarantee starts at delivery, return shipping is free, and you do not need to finish the bottle to qualify.
